What does waist circumference add after 40?

Brazil and the United States use waist measurements differently.
United States — 2026 obesity / CKM guidance
- The 2026 U.S. obesity standards screen adults at least annually with BMI and add waist-based measures in selected people to reduce underdiagnosis.
- For non-Asian adults with BMI 25.0–29.9 kg/m², waist-to-height ratio ≥0.50 or waist ≥35 in (88 cm) in women or ≥40 in (102 cm) in men can support an obesity diagnosis. For adults with an Asian background, lower BMI and waist thresholds apply: BMI 23.0–27.4 kg/m² with ratio ≥0.50 or waist ≥31 in (80 cm) in women or ≥35 in (90 cm) in men.
- Diagnosis is followed by evaluation and staging. A waist threshold is not, by itself, evidence of organ dysfunction or a complete treatment plan.
Brazil — Ministry of Health / ABESO 2026
- The Ministry of Health adult-obesity pathway lists waist >80 cm for women and >94 cm for men as parameters related to morbidity and mortality risk. These are risk-screening points, not a universal declaration of disease severity.
- ABESO 2026 emphasizes that cutoffs vary by population and ethnicity. It allows consideration of pharmacotherapy when increased waist and/or waist-to-height ratio coexists with adiposity-related complications regardless of BMI—but grades this recommendation IIb, level C.
- Brazil's highly admixed population requires caution when importing ethnicity-specific cutoffs. Measurement technique, complications, overall risk, patient goals, and access to evidence-based care remain central.
How to measure consistently
Use one defined protocol and repeat it the same way.
Locate the site
Use the midpoint between the lower margin of the last palpable rib and the top of the iliac crest for the WHO protocol; do not assume the navel is the correct site.
Prepare
Stand upright with feet together, arms relaxed, abdomen relaxed, and the measurement area uncovered or in light clothing.
Position the tape
Use a nonstretch tape, horizontal around the body, snug but not compressing the skin.
Read at a normal exhalation
Do not suck in the abdomen or hold the breath. Record in centimeters or inches without rounding to a preferred target.
Repeat
Take at least two readings and repeat if they differ meaningfully. For trends, use the same site, conditions, tape, and time pattern.
Escalate when context changes
Professional measurement is preferable when the result could change diagnosis or treatment, or when anatomy, pregnancy, edema, ascites, surgery, frailty, or severe obesity complicates interpretation.
