Liver enzymes and MASLD: do normal results rule out fatty liver?
ALT and AST can signal liver-cell injury. They do not measure liver fat directly, stage fibrosis, or reliably exclude clinically important liver disease.
The quick answer
No. Normal liver enzymes do not rule out MASLD, steatohepatitis, or advanced fibrosis.
ALT and AST are injury markers—not direct measurements of fat, inflammation, fibrosis, or liver function. MASLD is defined by hepatic steatosis plus at least one cardiometabolic risk factor after other causes and alcohol exposure are considered. In people at higher risk, current guidance uses a stepwise pathway: clinical context and routine tests, a first-line fibrosis score such as FIB-4, and—when indicated—a second test such as transient elastography or ELF.
Injury is not function
ALT and AST rise when hepatocytes are injured, but their concentration does not translate directly into disease stage. Albumin, bilirubin, and INR provide different information and are also influenced by non-liver factors.
Fat is not fibrosis
Steatosis means excess liver fat; MASH adds steatohepatitis; fibrosis is scar tissue. An ultrasound may show steatosis, but conventional ultrasound cannot reliably stage fibrosis and can miss mild fat.
Risk before certainty
FIB-4 combines age, AST, ALT, and platelets to estimate the probability of advanced fibrosis. It is a triage tool, not a diagnosis, and must be interpreted in the population and clinical setting in which it is used.
The central distinctionLiver enzymes ask whether injury may be occurring. Steatosis tests ask whether fat is present. Fibrosis tests ask whether scarring may be advanced. Those are three different questions.

Stepwise fibrosis assessment
FIB-4 sorts risk. It does not name a fibrosis stage.
FIB-4 = age × AST ÷ (platelet count × √ALT), using the units expected by the validated formula. A calculator reduces arithmetic and unit errors. Common adult pathways use the following categories, but age, acute illness, referral setting, and local guidance matter.
Lower risk in many adults
Advanced fibrosis is less likely. Reassess over time according to metabolic risk; a low result does not rule out steatosis or every liver disease.
Indeterminate
Use a secondary test—commonly vibration-controlled transient elastography (VCTE) or ELF—rather than treating the score as a diagnosis.
Higher risk
Advanced fibrosis is more likely and secondary assessment or specialist evaluation is appropriate. Confirmation is still required.

Each rating applies to the exact claim. Guideline recommendations combine diagnostic-accuracy evidence, prognostic cohorts, feasibility, and expert consensus; they are not all treatment trials.
Normal ALT and AST do not exclude MASLD, MASH, or advanced fibrosis.1,2,8
Aminotransferases can remain within the reference interval across the MASLD spectrum, including in people with type 2 diabetes and clinically significant fibrosis. Enzyme normalization also does not prove that fibrosis has reversed.
ALT and AST are markers of injury—not direct tests of liver function or fibrosis severity.1,5
ALT is relatively liver-specific, whereas AST is also present in muscle and other tissues. The magnitude of elevation does not reliably map to the amount of histologic injury or scarring.
MASLD requires hepatic steatosis plus at least one cardiometabolic risk factor.2,6,7
The 2023 multisociety nomenclature places MASLD under steatotic liver disease. Alcohol exposure and alternative causes still matter; MetALD identifies a defined overlap between metabolic dysfunction and higher alcohol intake.
Risk-based case finding is supported; blanket screening of every adult is not settled.1,2,8
AASLD and EASL pathways prioritize people with type 2 diabetes, obesity plus metabolic risk, imaging steatosis, or otherwise unexplained liver-enzyme elevation. The exact target groups and intervals differ by guideline and health system.
FIB-4 is useful for ruling out advanced fibrosis in appropriate low-prevalence settings.1,2,3,8,9
Its main strength is a relatively high negative predictive value at a low cutoff. A value below 1.3 in many stable adults lowers—not eliminates—the probability of advanced fibrosis.
FIB-4 of 1.3 or higher generally requires a secondary fibrosis assessment.1,2,3,4,8
VCTE or ELF is commonly recommended next. A score above 2.67 identifies higher risk, but FIB-4 alone cannot confirm cirrhosis or assign an exact fibrosis stage.
Conventional ultrasound can identify steatosis but a normal study does not reliably exclude it.1,2,4
Sensitivity falls with mild fat accumulation and obesity. Ultrasound findings also do not establish steatohepatitis and do not reliably stage fibrosis.
Liver biopsy remains the reference for histology but is not routine for every suspected MASLD case.1,2,8,9
It may be considered when noninvasive tests are discordant or indeterminate, alternative diagnoses remain possible, or histology would change a consequential decision. Invasiveness, sampling variability, and cost limit routine use.
A commercial ‘liver detox’ can be assumed to treat or reverse MASLD.1,2,5
The concept is not a diagnosis-based treatment strategy, and supplements may cause liver injury or interact with medications. Product claims cannot substitute for evidence tied to a defined intervention, population, and outcome.
Two native pathways
The scientific core is shared. Implementation differs in the United States and Brazil.
United States — AASLD
- Use targeted case finding in people with steatosis on imaging, unexplained liver-chemistry abnormalities, type 2 diabetes, medically complicated obesity, or multiple metabolic risk factors.
- In many primary-care pathways, FIB-4 <1.3 supports low-risk follow-up; ≥1.3 leads to VCTE or ELF; >2.67 increases concern and supports specialist referral or additional assessment.
- Use a cutoff of 2.0 for the lower-risk boundary in adults older than 65; avoid relying on FIB-4 during acute illness and recognize its lower accuracy under 35.
- Conventional ultrasound is not sensitive enough to exclude mild steatosis, and normal aminotransferases do not exclude advanced disease.
Brazil — SBD / hepatology pathways
- The 2026 Brazilian Diabetes Society guidance recommends FIB-4 in all adults with prediabetes or type 2 diabetes as initial advanced-fibrosis risk assessment.
- A FIB-4 result ≥1.3 leads to transient elastography in the SBD pathway; access, referral criteria, and follow-up must be adapted to the Brazilian health system.
- Brazilian guidance uses the Portuguese term DHEM for MASLD and emphasizes sequential noninvasive assessment rather than isolated liver enzymes.
- Alcohol exposure, viral hepatitis, medications, and other causes remain part of the differential diagnosis even when cardiometabolic risk is present.
Reading patterns
Combinations are more informative than one isolated result.
Normal ALT/AST + metabolic risk
MASLD and clinically significant fibrosis remain possible. Estimate pretest risk and consider a fibrosis pathway when the person meets guideline-based risk criteria.
Mild ALT/AST elevation + metabolic risk
MASLD may be plausible, but alcohol, medications and supplements, viral hepatitis, muscle injury, and other liver diseases still need consideration.
Predominant ALP elevation + GGT elevation
This suggests a cholestatic or hepatobiliary pattern and often directs evaluation toward imaging and medication or biliary causes rather than assuming MASLD.
AST higher than ALT
The pattern can occur with alcohol-associated disease, advanced fibrosis, muscle injury, and other conditions. The ratio is a clue—not an etiologic diagnosis.
Steatosis on imaging + low FIB-4
Fat is present while advanced fibrosis appears less likely at that time. Cardiometabolic risk still matters, and reassessment frequency depends on risk.
High FIB-4 or low platelets
Advanced fibrosis becomes more concerning, but age, acute illness, hematologic causes, and other explanations must be considered. Confirm with a secondary test and clinical evaluation.
High bilirubin, prolonged INR, low albumin, or decompensation signs
These findings can signal impaired function or advanced disease in the right context and require prompt evaluation; medication, nutrition, hemolysis, and other non-liver causes can also affect results.
Common interpretation errors
The most familiar shortcuts are often the least reliable.
The principal harm is false reassurance from normal enzymes—or false certainty from one abnormal number.
‘Normal enzymes mean no fatty liver’
MASLD, MASH, and advanced fibrosis can exist with normal ALT and AST.
‘The higher the ALT, the worse the fibrosis’
Enzyme magnitude and fibrosis stage do not move in a reliable one-to-one relationship.
‘High GGT proves alcohol is the cause’
GGT is nonspecific and can rise with cholestasis, medications, metabolic disease, and other exposures.
‘An ultrasound grades the fibrosis’
Conventional ultrasound detects visible steatosis and anatomy; elastography and other tools answer fibrosis-risk questions.
‘A high FIB-4 proves cirrhosis’
FIB-4 stratifies probability. It requires confirmation and cannot assign an exact histologic stage.
‘A detox supplement is harmless’
Supplements may lack efficacy evidence, interact with treatment, and themselves cause liver injury.

- Failure to evaluate alcohol, medication, viral, or biliary causes

Move from a liver panel to a structured clinical question.
- 01
Define the pattern: hepatocellular, cholestatic, mixed, or impaired synthetic/clearance context—and determine whether the abnormality is acute or persistent.

Review symptoms, prior results, alcohol exposure, metabolic risk, medications, over-the-counter products, supplements, exercise or muscle injury, and relevant family and infectious history.

Confirm unexpected abnormalities and investigate causes according to severity and context; do not delay urgent assessment for jaundice, confusion, bleeding, ascites, fever with pain, or marked enzyme elevation.

When MASLD risk is meaningful, document evidence of steatosis and use an appropriate noninvasive fibrosis pathway rather than relying on ALT or ultrasound alone.

Calculate FIB-4 only in an appropriate stable adult context; interpret age-specific limitations and move to VCTE, ELF, or specialist assessment when the pathway indicates.

What is MASLD?+
MASLD is metabolic dysfunction-associated steatotic liver disease: hepatic steatosis plus at least one cardiometabolic risk factor, with alcohol exposure and alternative causes still evaluated.
What is the FIB-4 score?+
It combines age, AST, ALT, and platelet count to estimate the probability of advanced fibrosis. It is best used as a first-line triage tool, not as a diagnosis.
Is an abdominal ultrasound enough?+
No. It can identify visible steatosis and structural findings, but it may miss mild fat and does not reliably stage fibrosis or diagnose steatohepatitis.
Does an elevated GGT mean alcohol damage?+
Not necessarily. Alcohol is one possibility, but GGT is nonspecific and should be interpreted with ALP, other liver chemistries, medications, metabolic risk, and clinical history.
Does everyone with MASLD need a biopsy?+
No. Most pathways begin with noninvasive tests. Biopsy is reserved for selected diagnostic uncertainty, discordant tests, suspected alternative disease, or decisions that require histology.
When can liver-test abnormalities be urgent?+
Jaundice, confusion, vomiting blood or black stools, new abdominal swelling, severe right-upper-quadrant pain with fever, or a major acute enzyme rise require prompt medical assessment.
