Vitamin B12 and folate: when a ‘normal’ result may not settle the question

B12 and folate participate in blood-cell formation and one-carbon metabolism. B12 is also essential to the nervous system. No single result should be interpreted without symptoms, blood count, exposures, and assay limits.

Start with the clinical question, then use the smallest test sequence that can answer it.

Total serum B12 or active B12 is generally the initial test. A clearly low result plus compatible features supports deficiency. An indeterminate result may require methylmalonic acid (MMA), while folate, blood count, smear, reticulocytes, kidney function, and cause-directed tests can clarify the pattern. Neurologic B12 deficiency can occur without anemia or macrocytosis.

Folate may correct the hematologic appearance while neurologic damage from B12 deficiency progresses.

Does metformin cause B12 deficiency?+

Long-term use can reduce B12 in some people; symptoms, anemia, duration, and risk determine testing.

Should everyone measure folate?+

No. Testing should answer a defined question such as macrocytosis, anemia, malabsorption, or dietary risk.

Is methylfolate better than folic acid?+

Not universally. Folic acid has established roles such as neural-tube-defect prevention, while methylfolate has not shown broad superiority for every person or indication. Dose, pregnancy, diagnosis, medicines, and B12 status matter.

Is high B12 dangerous?+

A high result often reflects supplementation but may require context; it is not itself a diagnosis of toxicity.
